COVID-19 Vaccine:

 

Please read the important information regarding Pfizer COVID-19 vaccinations for children ages 5-11 years.

 

  • Both vaccine appointments will be scheduled at the time of your call. They are scheduled 3 weeks apart on the same day and time.
  • We will be booking appointments in day\time order. If you need a later date\time, we will ask that you call the office at a later time to schedule.
  • If there is any vaccine hesitancy or concern about personal schedules, we ask that you please wait to schedule your appointment until you are sure of your decision and personal availability.
  • Electronic forms will be sent via email and\or text to you at 7 am the morning of your scheduled clinic. These forms must be completed electronically for each scheduled patient and submitted to our practice by 11am the morning of the clinic. Click here to review the forms ahead of time (please note: forms must be completed the day of the clinic, not before).
  • Please arrive at your scheduled time and park in the main parking lot.
  • Please do not call the office upon your arrival- you may enter directly through the normal entrance and check in at the desk.
  • Please understand that while we recognize that many children feel anxious while receiving vaccines, these clinics are scheduled to facilitate as many patients as possible and the nurse has a very limited amount of time in the exam room. We ask that you talk with your children ahead of time, and\or come prepared to assist the nurse in holding the patient during vaccination.
  • Depending on the patient’s medical history there is a 15-30 min required waiting period after vaccine administration.
  • Please note: during the clinic there will not be a provider available to see patients for add-on sick visits. We will be unable to administer additional school required vaccines, schedule future appointments, handle form or prescription refill requests, etc. Please call the office during regular business hours to address those needs.
  • If, for any reason, you will be unable to keep your scheduled appointment, please contact the office as soon as possible at 585-624-4520
