Financial Policies
- Co-payments\Deductibles are due at the time of your visit. If not paid at time a service, there is a $15 fee.
- You will be responsible for services not covered by your insurance plan.
- If you miss or do not cancel your appointment with 24 hours notice of your scheduled appointment time, you will be billed a $50 fee.
- A service fee of $20 will be charged for returned checks. You will need to pay the amount due, plus the service fee in a timely manner.
